You do not always need a referral to see a specialist in Cyprus. GESY beneficiaries can access outpatient specialists through a referral or, in many cases, directly with a different charge. A private appointment follows the practice’s booking and payment arrangements.
The important question is not simply “Does this doctor accept GESY?” It is “How is this particular appointment being booked?” That determines which rules and charges you need to check.
The three main routes to a specialist
- GESY with a referral: the HIO lists a standard €6 co-payment for an outpatient specialist visit with a valid referral.
- GESY direct access: for an ordinary outpatient specialist visit without a referral, the standard personal contribution is €25. This is a GESY route, not a private consultation fee.
- Private appointment: ask the practice for its consultation fee and any separate test or procedure charges. If using insurance, check the insurer’s referral and authorisation requirements too.
The €6 and €25 amounts are the HIO’s published standard visit charges, checked on 1 September 2026. They are alternatives for the relevant visit route, not automatically €25 plus €6. How a GESY specialist referral works
Your personal doctor is a usual starting point for a new concern. They assess what you need and whether a specialist referral is appropriate. Other referral routes also exist: the HIO’s specialist FAQ includes referrals from another outpatient specialist and from a hospital providing inpatient care.
A standard specialty referral does not normally mean you must attend a named doctor. HIO access guidance explains that the beneficiary chooses a contracted specialist in the relevant specialty and contacts the practice to arrange the appointment. Read the official specialist access guidance.
Before booking, have the practice check:
- That the referral is for the correct specialty or service.
- That it remains valid on the appointment date.
- How many covered visits or activities remain.
- Whether the appointment you want can use that referral.
Do not assume every referral has the same duration or visit allowance. Normal, long-term and activity-specific referrals serve different purposes. Use the details on your actual referral and ask the issuing clinician or HIO about anything unclear.
Can you see a specialist without paying the €25 direct-access charge?
One important exception is direct access to gynaecology and obstetrics for female GESY beneficiaries aged 15 and over. The HIO states that the standard €6 co-payment applies instead of the €25 personal contribution. Other condition-specific arrangements or exemptions should be checked for the individual beneficiary rather than assumed. This does not mean that every service offered by a specialist is automatically covered without a referral. In particular, the HIO distinguishes diagnostic services provided by the Special Group of Outpatient Specialists: direct access to those diagnostic tests without a referral is not covered under that rule.
If your booking is for a scan, test or procedure rather than a consultation, ask specifically about that service. “I can see the doctor directly” and “this test is covered” are different questions.
Private specialist appointments: ask about the whole visit
Many practices accept direct private bookings, but a clinic may still ask for a referral letter, previous results or an initial assessment for the service you request. An insurer can also have its own conditions even when the doctor will see you without a referral.
Ask for the consultation price before attending, then ask what it includes. A useful booking message is:
I would like a first consultation for [brief reason]. Is this appointment available through GESY or privately? What is the consultation charge, and would any tests or procedures be charged separately?
There is no reliable single national price for all private specialist consultations. Fees vary by practice and service. Avoid making a decision from an old price or another patient’s experience of a different procedure. Choose the right specialist, then confirm the appointment

Check their stated languages, services and contact details, then confirm current GESY participation or private availability directly. Tell reception whether you are booking for an adult or a child and whether you already hold a referral. Confirm the exact clinic location; a doctor may work from more than one setting.
Common referral questions
Can I change specialist after my first visit?
HIO guidance allows beneficiaries to change their chosen specialist. However, changing doctor does not create a fresh allowance of visits on the same referral. Have the new practice check the remaining entitlement before booking.
Does a follow-up automatically count as part of the first appointment?
Do not assume it does. Ask whether the follow-up uses another referral visit, needs a new referral or has a private charge. Also confirm who will explain any results and how you will receive them.

Before you book: one final check
Make sure you can answer four questions: Which doctor? Which appointment route? Which referral or authorisation? Which charge? Clear answers prevent confusion between a covered consultation, an additional service and a private booking.
